Integrated Care Partnership Board and Supporting Meetings Report of Warren Heppolette, Chief Officer Strategy, Innovation and Population Health, NHS Greater Manchester Decision: 1. That the contents of the report be noted. 2. That a terms of reference be drafted for the ICPB strategy meetings. 3. That a forward plan be drafted. 4. That eight strategy meetings be held annually. |

Primary Care and Live Well A joint report between NHS GM and Primary Care Provider Board Decision: That the GM Integrated Care Partnership Board: -
1. Note the central role of Primary Care in Live Well, its track record of innovation in this area, and emerging issues/considerations. 2. Note that the PCB has been commissioned to undertake a detailed engagement across Primary Care to support the Live Well design and implementation. 3. Note the proposed timescales of this engagement and its application to other system work. 4. Endorse that interim reports will be shared with the Live Well team and key stakeholder groups 5. Endorse that a full report is presented to the ICP by July 2025. |

Supporting People to Return Home from Hospital and to Live Well at Home Report of Colin Scales, Deputy Chief Executive, NHS Greater Manchester and Stephanie Butterworth, Tameside Council Director of Adult Services and Chair of GM ADASS. Decision: That the GM Integrated Care Partnership Board: -
1. Note the contents of the report, particularly the current challenges and good work underway to address them. 2. Note the opportunities for further collaborative improvement between health, care and wider public service to improve discharge from hospital, and build on our Home First approach, in the context of the GM Live Well model. 3. That a further paper focusing on admission avoidance in the context of the GM Live Well model is brought to a future ICPB Strategy meeting. |
